This WHO estimate covers anemia from all causes, not heavy menstrual bleeding alone. It establishes the scale of anemia while individual evaluation must identify menstrual, nutritional, gastrointestinal, inflammatory, genetic or other contributors. [5]

Heavy bleeding is defined by impact

NICE defines heavy menstrual bleeding as excessive menstrual blood loss that interferes with physical, social, emotional or material quality of life. [1] This avoids forcing people to measure blood volume at home. Clues include flooding, frequent product changes, bleeding through clothes or bedding, large clots, prolonged periods and changing normal activities to remain near a bathroom.

The pattern matters. Heavy regular periods, irregular bleeding and bleeding between periods can point toward different causes. Pregnancy-related bleeding, fibroids, adenomyosis, ovulatory dysfunction, endometrial pathology, medication effects and inherited bleeding disorders all require different consideration. Sudden severe bleeding with dizziness, fainting, chest pain or shortness of breath needs urgent assessment.

  • Describe frequency, duration, flooding and clots.

  • Note pain, pressure and pregnancy possibility.
  • Describe effects on work, sleep, exercise and intimacy.

Iron deficiency can exist before anemia

Repeated menstrual blood loss can deplete stored iron. A person may develop fatigue, reduced exercise tolerance, difficulty concentrating, headache, palpitations or restless legs before hemoglobin falls into an anemic range. A normal hemoglobin therefore does not always exclude iron deficiency. Ferritin and other iron studies may be appropriate when symptoms, diet, blood loss or clinical history raise concern.

Guidelines are inconsistent about routine ferritin testing in every person with heavy menstrual bleeding. A review of international guidance found substantial variation in recommendations for screening and treatment of iron deficiency. [4] This uncertainty supports individualized testing rather than ignoring symptoms. Inflammation can also raise ferritin, so results require clinical interpretation.

  • Hemoglobin reflects anemia, not total iron reserve.
  • Ferritin is useful but can be altered by inflammation.
  • Diet alone may not replace ongoing heavy losses.
  • The cause of bleeding and the deficit both need attention.

Investigate while starting appropriate symptom control

NICE recommends a full blood count for all women with heavy menstrual bleeding, in parallel with treatment. [1] History and examination guide whether first-line investigation should be hysteroscopy or ultrasound. Persistent bleeding between periods, risk factors for endometrial pathology, significant period pain, a pelvic mass or a bulky tender uterus can change the pathway.

ACOG lists fibroids, adenomyosis, ovulatory problems, bleeding disorders, some medicines and cancer among possible causes. [2] Adolescents with very heavy bleeding from menarche, easy bruising, frequent nosebleeds, prolonged bleeding after dental work or a family history may need assessment for a bleeding disorder. The test plan should be proportionate to age, symptoms and risk.

  • Do not postpone a blood count while waiting for imaging.
  • Choose imaging according to symptoms and examination.
  • Consider coagulation evaluation when history supports it.
  • Review anticoagulants and other relevant medicines safely.

Control bleeding with the person's goals in view

Treatment can begin before a final structural diagnosis when history suggests low risk, but red flags still require investigation. Options may include a levonorgestrel-releasing intrauterine system, tranexamic acid, nonsteroidal anti-inflammatory drugs, combined hormonal contraception or cyclical oral progestogens, depending on suitability and preference. [1] These options differ in contraception, side effects and effect on pain.

Fibroid size and location, adenomyosis, cavity distortion, fertility plans and prior response can change effectiveness. Procedural options may be considered when medical treatment is unsuitable, ineffective or not preferred. Endometrial ablation and hysterectomy affect future pregnancy fundamentally. Shared decisions should compare symptom benefit, recovery, risk, repeat treatment and reproductive implications.

  • Clarify whether contraception is desired.
  • Ask how quickly pregnancy is planned.
  • Agree a realistic bleeding and quality-of-life target.
  • Plan review instead of repeating ineffective treatment indefinitely.

Replace iron safely and check response

Oral iron is commonly used when deficiency or iron-deficiency anemia is identified, while intravenous iron may be appropriate when oral treatment is not tolerated, absorption is impaired, anemia is more severe or faster replacement is clinically needed. Specific preparation, dose and duration should follow local guidance and individual factors. Iron should not be prescribed indefinitely without confirming need and response. [1][4]

Follow-up can include symptoms, hemoglobin and iron stores at an interval suited to severity and treatment. A poor response may reflect continued blood loss, nonadherence caused by side effects, an incorrect diagnosis, malabsorption or another source of bleeding. Improvement in hemoglobin does not necessarily mean iron stores are fully replenished. [4]

  • Discuss gastrointestinal side effects and practical dosing.
  • Keep iron away from children because overdose is dangerous.
  • Recheck rather than assuming replacement worked.
  • Escalate if bleeding continues or symptoms worsen.

Measure recovery in daily life, not laboratory values alone

The treatment objective is restored function and acceptable bleeding, not only a normal report. A useful baseline includes days of bleeding, flooding episodes, missed activities, fatigue, exercise tolerance and pain, alongside laboratory findings. NICE explicitly prioritizes quality of life over focusing narrowly on blood loss. [1]

Population anemia statistics cannot diagnose an individual or prove menstrual causation. WHO emphasizes that anemia remains a major public-health concern and can impair well-being and productivity. [5] Clinically, the disciplined response is to identify the cause, stabilize urgent cases, treat the bleeding, rebuild iron when indicated and verify that the plan has worked.

  • Track bleeding burden and function together.
  • Confirm improvement in anemia and iron reserve when relevant.
  • Revisit the differential diagnosis after treatment failure.
  • Provide an urgent-care plan for severe bleeding symptoms.

What the evidence cannot yet answer

  • The global anemia estimate includes many causes and cannot quantify the share due to menstrual bleeding.
  • Clinical guidelines differ on routine ferritin screening and thresholds in heavy menstrual bleeding.
  • Ferritin may be misleading during inflammation, infection or chronic disease.
  • Treatment effectiveness varies with underlying pathology, anatomy, adherence and reproductive goals.
