The transition is a clinical window, not a disease
Menopause is reached after 12 months without menstruation when there is no other cause. Perimenopause begins earlier, with cycle variation and symptoms. The transition is normal, but changes in estrogen, body composition, lipids, vascular health and bone remodeling can reveal or accelerate health risks. The AHA identifies it as a window for earlier cardiovascular prevention. [1]
Symptom care and prevention should be connected. A visit for hot flushes, sleep disruption or bleeding change is also an opportunity to review blood pressure, lipids, glucose, smoking, activity, nutrition, fracture history and reproductive history. It should not become a checklist that obscures the person's immediate concern.
- Clarify stage from cycle history when possible.
- Investigate abnormal bleeding appropriately.
- Record premature or early menopause.
- Include pregnancy complications in cardiovascular history.
Bone loss reflects estrogen change and baseline risk
Estrogen deficiency contributes substantially to postmenopausal bone loss. The Menopause Society recommends assessing fracture risk, reducing modifiable risks and using pharmacologic treatment for people at significant osteoporosis or fracture risk. [3] Bone-density testing is not determined by menopause symptoms alone and should follow age, fracture and risk-based guidance.
Risk factors include prior fragility fracture, low body weight, family history, glucocorticoid exposure, smoking, excess alcohol, malabsorption, rheumatoid arthritis and premature ovarian insufficiency. Resistance and weight-bearing activity, adequate protein, calcium and vitamin D, and fall prevention support bone health. They do not replace indicated osteoporosis medication.
- Ask about fractures and height loss.
- Review medicines and conditions that affect bone.
- Use validated fracture-risk assessment where appropriate.
- Repeat bone density only when the result can change care.
Cardiovascular risk can accelerate across midlife
Longitudinal studies show adverse changes in LDL cholesterol, body composition, metabolic syndrome risk and vascular measures during the menopause transition. [1] Earlier natural menopause is a marker of higher cardiovascular risk. These are population findings, not proof that estrogen change is the only cause or that every individual experiences the same trajectory.
Risk assessment should use standard prevention principles: blood pressure, lipids, glycemia, smoking, activity, sleep, weight trajectory, kidney disease and family history. Pregnancy complications such as preeclampsia and gestational diabetes add important context. Menopause hormone therapy should not be prescribed solely to prevent cardiovascular disease.
- Measure blood pressure accurately.
- Assess lipids and diabetes risk according to guidelines.
- Treat established risk factors directly.
- Do not confuse symptom relief with proven cardiovascular prevention.
Hormone therapy decisions depend on indication, timing and formulation
Hormone therapy is the most effective treatment for vasomotor symptoms and can prevent bone loss while it is used. The Menopause Society states that benefit-risk balance is generally favorable for appropriately selected symptomatic women younger than 60 or within 10 years of menopause onset who lack contraindications. [4] That population statement still requires individual assessment.
A person with a uterus needs endometrial protection with systemic estrogen. Oral and transdermal routes have different risk considerations. Prior breast cancer, unexplained bleeding, thromboembolism, stroke, liver disease and cardiovascular status can change suitability. NICE recommends communicating absolute risks and tailoring decisions, including route and regimen. [5]
- Name the treatment indication.
- Use the lowest effective regimen consistent with goals.
- Protect the endometrium when required.
- Review benefit, adverse effects and changing health status.
Premature and surgical menopause require distinct attention
Menopause before age 40 and early menopause before age 45 are linked with greater bone and cardiovascular risk. Abrupt ovarian loss after bilateral oophorectomy produces a different hormonal change from natural menopause. The AHA emphasizes earlier age as a risk marker, while menopause guidance generally supports hormone replacement until the usual menopause age when no contraindication exists. [2]
These individuals should not have evidence from older postmenopausal populations applied without context. Care should address symptoms, bone density, cardiovascular risk, sexual health, mental health and fertility implications. Cancer-related surgical or medical menopause requires coordination with oncology because tumor biology and ongoing treatment can alter options.
- Document age and cause of ovarian loss.
- Assess bone health earlier when indicated.
- Distinguish replacement from late initiation.
- Coordinate complex cancer-related decisions.
Build a prevention plan that can be followed
A useful plan sets actions and review intervals. It might include home or clinic blood-pressure follow-up, lipid and glucose assessment, progressive strength training, nutrition review, smoking cessation, fall-risk reduction and treatment for bothersome symptoms. The plan should match ability, disability, culture, resources and personal priorities rather than assume one ideal lifestyle. [1][2][3][5]
Evidence is strongest when standard cardiovascular and osteoporosis prevention is applied according to risk. Evidence is weaker for using supplements, compounded hormones or unvalidated tests to personalize broad disease predictions. Midlife care is most sophisticated when it reduces fragmentation: one coherent story linking symptoms, reproductive history, bone, heart and the years ahead. [1][3][4][5]
- Choose actions with named owners and dates.
- Track strength, function and adherence, not weight alone.
- Reassess symptoms and preventive risk together.
- Avoid unvalidated hormone panels for disease forecasting.
What the evidence cannot yet answer
- Menopause-transition studies are largely observational and cannot isolate all effects of ovarian ageing from chronological ageing.
- Hormone-therapy findings vary by age, timing, route, formulation and baseline risk.
- Evidence does not support using hormone therapy solely for primary or secondary cardiovascular prevention.
- Bone-density testing and drug thresholds differ by jurisdiction and individual risk.
