Preeclampsia is more than high blood pressure
Preeclampsia is a hypertensive disorder that usually begins after 20 weeks of pregnancy and can involve the kidneys, liver, brain, blood and placenta. It may present with proteinuria or other organ dysfunction. WHO estimates that it affects 3% to 8% of women who give birth worldwide. [1] Symptoms can be absent, which is why routine blood-pressure assessment matters.
Severe headache, visual change, upper abdominal pain, shortness of breath, sudden swelling or feeling acutely unwell requires urgent evaluation during pregnancy or postpartum. Preeclampsia can first appear after birth and ACOG notes that postpartum onset can occur up to six weeks. [4] Normal delivery does not end the immediate risk.
- Know the individual's blood-pressure action threshold.
- Provide a postpartum warning-sign plan before discharge.
- Use urgent assessment for neurological or respiratory symptoms.
- Do not rely on swelling alone to diagnose or exclude disease.
Acute care prevents maternal and fetal harm
Management depends on severity, gestational age and maternal and fetal status. It can include close monitoring, antihypertensive medication, corticosteroids when preterm delivery is anticipated and magnesium sulfate to prevent eclamptic seizures. WHO reports that magnesium sulfate can reduce eclampsia risk by more than half in women with preeclampsia. [1] Delivery is the definitive end to the pregnancy-related placental process, but postpartum monitoring remains essential.
Prevention strategies for high-risk pregnancy include low-dose aspirin and calcium supplementation in settings of low dietary calcium intake according to WHO guidance. [1] These are not universal self-care recommendations. Eligibility, dose and timing should be determined with antenatal clinicians because risk profile and local guidelines matter.
- Start preventive medication only on clinical advice.
- Treat severe hypertension urgently.
- Monitor maternal organs and fetal wellbeing.
- Continue surveillance after delivery.
Pregnancy acts as a cardiovascular stress test
The AHA recognizes hypertensive pregnancy disorders as adverse pregnancy outcomes associated with later hypertension, coronary disease, stroke, heart failure and other cardiovascular outcomes. [2] Some risk reflects pre-existing factors, while pregnancy may also reveal vascular vulnerability. Either way, the history identifies a population that can benefit from prevention.
ACOG describes pregnancy complications as a natural stress test but notes that later risk is not consistently communicated. [5] A preeclampsia diagnosis should therefore remain visible in the lifelong medical record. It should appear in primary care and cardiovascular histories rather than disappearing into an archived obstetric note.
- Add preeclampsia to the permanent problem or history list.
- Communicate severity and gestational timing when known.
- Transfer the record from obstetric to primary care.
- Include recurrence counseling before another pregnancy.
The postpartum handoff needs named actions
Postpartum care should include timely blood-pressure review, medication adjustment, symptom assessment and a clear route for urgent escalation. Persistent hypertension requires ongoing management. People with early or severe preeclampsia, persistent hypertension, diabetes, high cholesterol or cardiac symptoms may need specialist cardiovascular assessment. [4]
A handoff is not complete when advice says only to see a doctor. It should name who will review blood pressure, when medication will be reassessed and how contraception or future pregnancy plans affect drug choice. Remote blood-pressure monitoring can support follow-up when clinically governed, but it needs validated devices, thresholds and a response team.

- Reconcile medicines and breastfeeding considerations.
- State urgent thresholds and contact routes.
- Arrange primary care continuity before obstetric discharge.
Long-term prevention uses established cardiovascular care
Follow-up should assess blood pressure, lipids, glucose, smoking, kidney health, physical activity, sleep and family history. The right interval depends on baseline findings and local prevention guidance. A history of preeclampsia changes risk context but does not justify unvalidated scans, supplements or biomarkers.
Prevention includes evidence-based treatment of hypertension, diabetes and dyslipidemia, plus sustainable nutrition, physical activity and smoking cessation. Lactation may be associated with lower later cardiometabolic risk, but feeding decisions are personal and constrained by health, medication, milk supply, work and preference. It should not become another source of blame. [2]
- Treat measured risk factors directly.
- Support activity and nutrition without weight stigma.
- Include kidney assessment when clinically indicated.
- Do not frame later cardiovascular disease as inevitable.
Health systems should make the signal actionable
A reliable system can automatically carry the diagnosis into discharge summaries, primary-care registries and future preconception care. It can prompt blood-pressure follow-up and standard risk assessment while avoiding alarmist language. Measures should include completed handoffs and controlled risk factors, not alert generation alone.
Disparities in adverse pregnancy outcomes and cardiovascular care mean implementation must be audited across race, ethnicity, geography and socioeconomic conditions. [3] The most important insight is practical: pregnancy history is cardiovascular history. Preserving it creates an earlier opportunity to prevent disease without claiming that one pregnancy determines a person's future.
- Use structured pregnancy-history fields.
- Audit completed postpartum and primary-care follow-up.
- Track equity in monitoring and treatment.
- Include the history in future pregnancy planning.
What the evidence cannot yet answer
- Long-term risk estimates are observational and vary by preeclampsia severity, recurrence, age and population.
- Shared cardiovascular risk factors can contribute both to preeclampsia and later disease.
- Optimal timing and intensity of every long-term test after preeclampsia are not fully established.
- Remote monitoring improves measurement only when a clinical response pathway is available.
